You may then fill out the required paperwork by printing and completing these linked forms. Then
scan the forms and return them by email. Or just mail them with a stamp.
(If you don't have a scanner, just take
photos of the forms and email the photos to us.)
We will respond to your order request by email or with a phone call (usually
within 24 hours. If you do not hear from us, please check your spam folder for
the response, or call to make sure we received your order.
If your order delivery date is less than 7 days away, be sure to call,
even if you have mailed or emailed a completed Order Form.
If you are faxing forms, please call or send an email to let us to
know to look for your fax.
